Trans Woman Becomes First Recorded to Breastfeed

It was not even possible through the gender-reassignment surgery like a breast augmentation or a vaginoplasty.

The three-and-half month treatment included hormones, a nausea drug and a breast pump to stimulate milk production.

Following months of hormone therapy previous year, doctors say she might be the first reported transgender woman in academic literature to breast-feed, according to a case study published last month in the peer-reviewed journal Transgender Health.

"We want to present our patients with the full range of reproductive choices, and this is one step closer to that", Dr. Tamar Reisman, an endocrinologist at Mount Sinai hospital's Center for Transgender Medicine and Surgery in NY, told the Washington Post. "At six weeks, the patient began supplementing breastfeedings with 4-8 oz of Similac brand formula daily due to concerns about insufficient milk volume".

The woman had been already been on hormone replacement therapy since 2011 so Reisman and Goldstein increased her hormone intake. Now, in the wake of this success, the world of transgender health has been changed forever.

The FDA explains that there are "serious risks associated with domperidone" and they took action on the drug because of the risks associated while patients were using it to enhance breast milk production.

Paediatricians said the child's growth and feeding were both normal. Though used in many countries, including in Britain, it has been banned in the United States because of Food and Drug Administration concerns about its associations with cardiac arrest and sudden death. Dr Reisman's findings have been published in the journal Transgender Health and notes: "We believe that this is the first formal report of induced lactation in a transgender woman".

After the initial trial and recommended dosage, by her first follow-up, she was able to express droplets of milk. As of 2015, Health Canada has not received any reports of serious heart-related adverse effects in anyone of childbearing age taking domperidone.

By the third month, two weeks ahead of the baby's due date, she was producing eight ounces of milk a day, and was able to lower her progesterone and estradiol doses.

In addition to the domperidone, the patient took micronized progesterone and estradiol, while also using a breast pump for five minutes on each breast three times a day. She continued breastfeeding along with formula feeds. Deutsch, also a transgender woman with a 6-month-old child, said she understands transgender mothers, but trying to induce lactation is "not something I would do".
